Please use this form to commend the 9-1-1 center or its employees, register complaints, ask questions, or make suggestions. We will respond as soon as possible. You can also contact us by dialing 3-1-1 during regular business hours.

All complaints will be investigated. If you are filing a complaint, you will be given the name, phone number, and email address of an employee who you can contact about the investigation. We will also let you know the approximate timeline of when you can expect future updates regarding the investigation into your complaint. Upon completion of the investigation, you will be notified of its result.

This form is for feedback regarding Manatee County's 9-1-1 or 3-1-1 services only. If you would like to provide feedback regarding other county services, including EMS, please contact those divisions directly. We cannot respond to feedback that does not relate to 9-1-1 or 3-1-1.

If you feel you have been injured or sustained property damage as the result of the activities of Manatee County Government, you must complete and submit the Citizen Property/Injury Incident Form

If you need to view an elevation certificate for a specific address, please complete the form below, and the certificate will be emailed to you, if available.

If a Flood Elevation Certificate does not yet exist for the address, you can submit an application for Flood Elevation Determination through our Online Services platform.

If you prefer to complete this form in person, you may still visit our Permitting Office on the 4th Floor of the  Manatee County Administration Building.

If there is an item that Manatee Library does not own and you would like to suggest that it is purchased for the system, please fill out the form below.

All suggestions will be considered based upon cost, item format, publication date (items less than one year old), availability, and suitability to a public library's collection including, but not limited to, these factors: current usefulness, permanent value, popular demand, newsworthiness, high standard of quality, critical reviews, and reputation and authority of the publisher, author, or producer.

We recommend requesting most items that are over one year old through Interlibrary Loan. Items that are part of our standing order (popular authors whose books we automatically order) will not be eligible as a suggested purchase. 

Please visit OverDrive/Libby or Hoopla to view our collections of eBooks, eAudiobooks, streaming video, and streaming music. 
Schedule our Pop-Up Library to visit your care facility or retirement community in Manatee County. Our monthly Lobby Stop provides residents with assistance in registering for a Library card, accessing e-resources and e-reading materials, and selecting physical materials to check out from our traveling library collection.

If conditions requiring a backflow preventer are eliminated (i.e. in-ground irrigation system removed and no other conditions requiring backflow preventer exist at the property), you may submit a Petition to Remove a Backflow Prevention Assembly to the Cross Connection Control Office. Petitions will be reviewed within 30 days. Backflow preventer shall never be removed without approval.
