Quarterly Program Outcomes Measures Report
This report must be complete for all programs funded through Manatee County Government's Community Services Division, regardless of the funding source. The report must report the outcomes for the quarter per program. This report is due for each funded program on or before the 30th of the following months (for the quarter ending the prior month): Jan, April, July, and October. These reports must be completed and submitted through the online data management system (ODM)
Feedback will be provided by Shelley Robertson within a week of submission due date. Please read the feedback and take the appropriate action.
Quarterly Financial Report
This report must be completed and submitted for each program funded through Manatee County Government's Community Services Division, regardless of the source. The report must show all expenditures and revenues for the quarter. This report is due for each funded program on or before the 30th of the following months (for the quarter ending the prior month): Jan, April, July, and October. This report is accessible through the online data management system (ODM).
Be sure to report all revenues and expenditures accurately for each program.
Quarterly Non-Profit Agency Funding Report
This form is utilized to report any alternative funding sources for which the agency has applied to either offset or replace the funding received from the county for each program. This report is due for each funded program (regardless of county funding source) on or before the 30th of the following months (for the quarter ending the prior month): Jan, April, July, and October. You must report what you are doing to obtain alternative funding for the program as well as the results of obtaining such funding. You may include special events, donations or bequests if they are specifically dedicated to the program. This report will be accessible on the online data management system (ODM).
